1. Track your symptoms for 7 days

Use this diary to record chest or abdominal pain and related symptoms for one full week (Sunday–Saturday).


Open the diary below, check “Week commencing” and select today’s date. Record meals and drinks, medicines and doses, bowel movements, mood and well-being. Add each pain episode, associated symptoms and effects on your day. Record days without pain too.


Return each day using the same browser on the same personal device. Entries save automatically there; they are not sent to the clinic. Avoid private browsing or clearing browser data, and protect your device with a passcode.


After all 7 days, review each date and select “Print / save PDF”. Choose “Save as PDF” in the print menu and save the weekly summary. On a phone, use the print/share options to save a PDF to Files or Downloads. Check the dates before uploading in Step 2.


This diary is not monitored. Call 911 for severe or persistent chest pain, major breathing difficulty, fainting or other emergency symptoms.

Prefer a PDF or paper diary?

Choose either version below and record a full week.


Fillable PDF: download and open in a PDF reader that supports forms, such as Adobe Acrobat Reader. Type into the fields, save a copy, then reopen it to check your answers were saved.


Printable blank PDF: print and fill in by hand. To send it, scan every completed page or take clear photos in good light. Include the whole page and check that all writing is readable.


Upload the completed PDF, scan or photos through the Hushmail button in Step 2. You can also keep the diary for yourself and bring the saved file or paper copy to discuss at your appointment. Downloading or completing a form does not send it to the clinic.

2. Upload your weekly PDF securely

Already completed your week? Have your saved PDF ready, then use the button below to open our Hushmail Secure Messaging and File Transfer form.


1. Enter your first and last name, email address and phone number.

2. In “Message”, write: “Weekly symptom diary — week commencing [date]”.

3. Select “Add file” and choose your weekly PDF from Files or Downloads. Check that you attached the correct file.

4. Select “Submit” and wait for the form’s confirmation before closing the page.


Saving the diary does not send it to the clinic; you must upload and submit the PDF. Keep a copy until the clinic confirms receipt.


If you cannot save or upload the PDF, call (705) 645-2281 Ext 3 for help. This form is for routine communication, not urgent care.
